I, the undersigned and/or parent/guardian of the individual(s) named above, do hereby agree to indemnify and hold harmless Kids Impact Community, and its agent from and against any and all liability resulting from participation in the project listed above. I understand that the project (s) in which I am enrolling, like all activity programs, has some inherent risk, for which I agree to assume the liability. Furthermore, the individuals named herein are in good health appropriate for the activities in which they will be participating.
In order to ensure meals are safe to be distributed throughout our community, I agree to:
1) Follow USDA guidelines to fully clean then sanitize the surface prior to meal preparation
2) Follow the Wisconsin Department of Agriculture, Trade and Consumer Protection guidelines on washing hands with soap and water for at least 20 seconds
3) Follow the WI DATCP rules on no bare hand contact with ready-to-eat foods ie. wear gloves or use tongs. When using disposable gloves, refer to these guidelines.
For KIC promotion purposes, KIC will be requesting submission of photographs of participants to share on our social media sites. No names or any other identifying information will be shared if the photograph is used, unless requested. I hereby release and discharge KIC from any and all claims and demands arising out of or in connection with the use of the photographs I submit, including without limitation any and all claims for libel or invasion of privacy.
